500 Internal Server Error - Beta Testing Form

Hey, so I was trying to sign up for beta testing through the application
(https://surveys.adguard.com/en/beta_testing_program/form.html) and it gave me this weird error, "500 Internal Server Error." My connection is good everywhere else.

If it helps, I'm using Opera GX, Windows 10 Pro
